Add options to self-enroll creation page to approve or be notified
On the self-enroll creation page add the follow notification options:
1 - Ability to approve/deny enrollment (instructor receives email requesting enrollment)
2 - Notification only (auto approved) (email to instructor or new enrollee)
3 - No notification (just like it is now)

On the self-enroll page, a notification check box has been added to notify the instructor when a student self-enrolls. Thank you for the improvement suggestion.
